High PSA of 9.28
My father in a routine psa test found psa to be 9.28. Mri gave pirads4 18*11mm nodule. I am also attaching mri report. Doctor is asking for biopsy. Can pirads4 be benign? What are chances? And why his dre was painful?

PIRADS 4 means high likelihood of clinically significant prostate cancer. So go for prostate biopsy. Painful DRE can be due to discomfort associated with DRE itself or prostatitis also.
