If he has no other medical issues and no symptoms of heart disease then you can try lifestyle modifications for couple of months. Exercise regularly for at least 30 minutes a day which includes either brisk walk or jogging or cycling. Keep BMI <25. Avoid junk food and red meat. Have high fiber diet. Repeat lipid profile after 2 months

Ldl is 181 which is quite high if there is family history of cardiac illness than it is be treated
