Treatment of hiatus hernia depends upon its size and symptoms. As you have severe GERD then permanent solution is surgery which is done now a days laparoscopically. If you don't opt for surgery then medication will be required for long periods may be life long
Next Steps
Avoid hot and spicy thing including tea and coffee. strictly no smoking or alcohol. Include fruits in your dinner preferably dinner should be only of fruits and salad. This along with medicine will subside your symptoms significantly
