Hello
First of all i would like to really appreciate you for listing your problems in such a clear manner. The condition that you have described needs treatment in the form of medicines and counselling. A combination of both will lead to improvement. I think you must have already been on medications. You need to consult a psychiatrist who can review the treatment and suggest the best options ahead. With treatment you should get better.
Feel free for any further clarifications.
Regards.
