Hard breast severe pain fluid coming out

Asked for Female, 68 Years

My right breast became hard since last day, severe pain and little amount of fluid coming out. Around 15 years earlier , I have experienced such fluid leaking out like milk / puss , got complete check of that fluid and breast, one small gland observed but the than Doctor said not to worry. Please advise
