Your symptoms suggest a recurrence of H. pylori-associated erosive gastritis, which can happen if the bacteria are not completely eradicated or if there are additional factors contributing to gastric irritation. The black stool indicates upper gastrointestinal bleeding, likely from the inflamed stomach lining. Next Steps: 1. Confirm Eradication – A stool antigen test or urea breath test should be done at least 4 weeks after completing triple therapy to check if H. pylori is still present. 2. Extended Treatment – If H. pylori persists, a second-line antibiotic therapy with a different combination may be needed. 3. Diet & Lifestyle Changes – Avoid spicy, fried, and acidic foods. Reduce caffeine, alcohol, and smoking (if applicable). Eat small, frequent meals and avoid late-night eating. 4. PPI and Mucosal Protectors – Continue proton pump inhibitors (PPIs) like omeprazole or pantoprazole for a few weeks under medical supervision. Medications like sucralfate may help heal the stomach lining. 5. Follow-up Endoscopy – If symptoms persist, another endoscopy may be needed to assess the severity and rule out complications like ulcers. Since your symptoms have returned, I strongly advise a proper follow-up consultation to reassess your condition and adjust treatment accordingly.
