To transition in to woman, you first need a psychiatric assessment, which would ascertain the diagnosis of gender dysphoria, other psychiatric conditions, the social, psychological and financial support you have at your disposal to go through the transition process. Once the decison has been made for you to go ahead with the transition,  you would need to start living in all social settings, dressed as a woman for a year. During this one year process, you will be referred to an endocrinologist for hormone therapy. If everything goes well, and depending on your choice, a referral to a plastic surgeon for cosmetic  and even gender reassignment surgery will be made. This while process will take a year or two and you would need constant psychological support during this period. Hormone therapy itself can be quite challenging, as it can lead to psychological and physical side effects. To summarize, you first need a psychiatric assessment and based on that assessment, a team of doctors will guide you through the process of transition, which will go on for years.
