​​Dear Doctor, ​I need guidance for a persistent UTI and severe vaginal discharge/odor. ​Symptoms: Burning urination, sharp urethral pain, hot urine feeling, frequent urination, lower abdominal pain, sticky white discharge, and strong odor on my underwear. ​History: Prescribed multiple oral antibiotic courses (Cetil, Norflox TZ, Septran DS, Nitrobest) and vaginal suppositories (Candid V3/V6, Candel V6), but symptoms have not improved. ​Urine Culture Results: E. coli isolated. ​Resistant to: Nitrofurantoin, Cefixime, Levofloxacin, Fosfomycin, Amoxicillin-Clavulanic acid. ​Sensitive to: Meropenem, Ceftriaxone, Gentamicin, Cefoperazone-Sulbactam, Amikacin. ​Questions: ​Since the infection is resistant to oral antibiotics, do I need immediate IV antibiotic therapy (e.g., IV Meropenem or Ceftriaxone)? ​Is the panty odor/wetness due to a concurrent vaginal infection or the resistant UTI? ​What is the correct combination of IV antibiotics and vaginal treatment to resolve bot
