This may be a mild tight foreskin (phimosis). It can cause discomfort, tearing, or difficulty during sex. Consult for evaluation; treatment may include stretching exercises, medicated cream, or circumcision if needed.

Based on your description, your foreskin can be retracted behind the glans when the penis is flaccid, but it tends to get stuck behind the glans and needs to be manually pulled forward. During erection, the foreskin retracts only partially, exposing about 40–50% of the glans. This suggests that you may have a mildly tight foreskin (mild phimosis) or a relatively tight foreskin opening. This is not considered completely normal foreskin mobility, but it is also not necessarily a serious problem. The main concern is that if the foreskin becomes trapped behind the glans and cannot be brought forward easily, it could potentially lead to paraphimosis, a condition in which the retracted foreskin causes swelling and becomes difficult to reposition. During sexual intercourse, a tight foreskin may sometimes cause discomfort, pain, small tears, or difficulty with full retraction.
Next Steps
Since you are already able to retract the foreskin to some extent, avoid forcefully pulling it back, especially during erections. Observe whether you experience pain, tightness, cracking, bleeding, swelling, or difficulty during sexual activity. If the foreskin consistently gets stuck behind the glans, it is advisable to have the condition evaluated before it becomes more troublesome. The fact that the foreskin opens only partially during erection suggests that a clinical examination would be useful to assess the degree of tightness and determine the most appropriate management approach. Many men with mild foreskin tightness are able to have normal sexual intercourse, but discomfort and complications are more likely if the tightness progresses. Health Tips
Maintain good genital hygiene and gently clean beneath the foreskin as much as comfortably possible. Never leave the foreskin retracted behind the glans for prolonged periods if it tends to get stuck. If swelling, severe pain, discoloration of the glans, or inability to pull the foreskin forward occurs, seek medical attention promptly. Early evaluation of foreskin tightness can help prevent discomfort and allow for a more confident and comfortable sexual life. Thank you
