Fever with recurrent chills and rigors
30yr old male running high grade fever with chills and rigors recurring every 6-7 hrs, rapid diagnostic test for falciparum malaria is positive, started on oral artesunate lumefantrin, and primaquine. Taking Dolo 650 for fever, on CBC- WBC count 4000, hb 13, platelet 1.5lac . Initially BP was 94/50 which improved after IV ringer lactate , since the rigors are recurring every few hrs , do I need to consider injectable artesunate??

You can give him iv paracetamol if the fever is not reducing. Please admit him as he will require continuous monitoring.

He needs to be admitted if he s not already.. continuous monitoring and parasite index is required..
