Hi...it's very important to know the age and sex of the patient.Fever with chills would usually suggest a stream infection(blood,urine or pus somewhere in the body).Malaria needs to be ruled out as well.Then again, an evening rise of temperature for around 3 months, which is low grade should also prompt investigations to rule out tuberculosis However, intricate details and detailed history is mandated to come to a proper diagnosis .
