My wife had anomaly scan and all the heart parameters are fine, but fetal radiologist has called for Fetal Echo, is this required, my gynea has mentioned intracardiac foci without anything in anomaly report- is this alarming, my wife mean utery pi is 98 percentile she is in her 20 weeks

An isolated intracardiac echogenic focus (ICEF) is usually a benign finding and not alarming if there are no other abnormalities on the anomaly scan. Fetal echo is advised only to rule out subtle heart defects as a precaution, especially if the radiologist noted anything. Your wife’s mean uterine artery PI being at the 98th percentile indicates higher risk for placental insufficiency or growth restriction, so close follow-up, blood pressure monitoring, and growth scans are important. Nothing alarming right now, but regular follow-up is necessary.
