It seems to be clinical depression. It needs to be treated asap otherwise it may get complicated. It can be well treated with counseling sessions and homeopathic medicine effectively. You need an expert psychologist.

Ma'am, you have mentioned your height as 5 ft 2 inches and weighing 86 kgs. The BMI (body mass index) is 34.4. It shows obesity. It is one of the reason for being sleepy. The normal range for BMI is 25. So, the weight should be around 60-65 kgs depending upon body factors. Maintaining a healthy weight is also important for your heart health. The sleepy feeling and laziness also results into negativity. My suggestion is as follows: 1. Regular exercise to loose weight. 2. Reduce oily, fatty foods (don't stop at once but reduce it step-by-step). 3. Consult a dietician (if you feel the need). 4. Meditate regularly to keep your mind fresh. 5. Consult after a week, then at fortnight, then at month later to keep track the changes. If above does not helps in refreshing your mind or you need any kind if mental support then consult a psychologist. All the best. Take care.
Next Steps
Exercise and meditation. Control on diet.
Health Tips

Exercise- Light; 3 days a week on alternative days 4. Nutrition composition- 20% protein, 30% fats and 50% carbohydrates 5. Calorie intake- 2000 cals/day 6.
