Try a trial course of ppi and prokinetics. Then stop by he medication and assess your symptoms. If symptoms continue then get esophageal manometry, 24 pH metry and endoscopy done. Based on how lax Les is and how severe reflux is, you can take a call whether to go for surgery or continue medication.
