Ectopic Pregnancy Treatment
The latest USG performed on 31st Oct suggests of an likely unruptured right tubal ectopic pregnancy without obvious periadnexal fluid.. Also as per USG Report right adnexa is bulky and shows a solid lession measuring  11MM * 14MM... Her LMP was 08th September and pregnancy tested positive on 15th Oct.. latest beta HCG done on 29th with count as 5,226.. There were multiple USG done on 22nd, 25th, 29th  but there were no evidences of ectopic observed, also multiple beta HCG tests done as well however HCG count didn't double as in the case of intrauterine pregnancy Now doctor has given methotrexate 50 mg dose on 31st Oct as the size is less than 3cm; would there be any requirement of a laproscopic surgery in this scenario required ?? Also my wife's left tube is blocked what are the chances of  pregnancy via IVF with her own eggs ???

Yes there is chances of failure of methotrexate therapy. Ectopic pregnancy can rupture and it needs surgery then. Successful methotrexate therapy will decrease the beta hcg level and ectopic mass will shrink in size. IVF with own eggs depends on AMH level and antral follicular count done on day 2 of her periods. At bhCG more than 1000 transvaginal scan should definitely pick up an ectopic mass If the titers  are not dubbing in 48 hours it is indicative of an ectopic pregnancy Yes methotrexate as a management of ectopic pregnancy comes with the failure rate in that case laparoscopic surgery may b required chances of pregnancy with IVF with own eggs  will depend upon the amh values
