Dropping eyelid
One eye seems little small than other. He is 4 months old and the problem is right from birth.
And as per my search it is dropping eyelid please suggest.

Show to Ophthalmologist if it's covering the cornea..then he will have to get operated early.. if it's not covering the cornea fully, then we can wait until he is around 5 year old...get eye testing done regularly.
