It looks you have gastritis (most probably Due to H.Pylori infection) plus also you have reflux disease also!
Next Steps
You will need to take H.Pylori eradication regime for 14 days plus you will have to make some life style changes(like avoiding spicy n oily foods,chocolates and no food for about 3 hours before Bed) and also long term PPI.
