Do I need extraction?
I went for jaw pain to dentist and spotted tooth#46 hurts for touching, tapping and biting pressure on it.So suggested root canal(xray normal) and did 2 years ago. No relief. Same jaw pain and neck pain.Same tapping and biting pressure on it till now.Had bite grind down on all teeth since they suspect my bite is the reason for the pain.No changes&bite changed now. Went to another Endo. He suggested and did retreatment RCT on January this year. No relief. Same symptoms of touching,tapping and biting pressure pain on it.No changes.Dentist wanted me to extract this tooth.As they suspect this tooth could be the reason for my constant pain in jaw under ear and neck.I saw ENT,OMFS,TMJ specialist, neurologist etc.No definite answer and relief yet.This tooth and gum hurts Immediately like throbbing underneath like sensitive den pressure like jump when it's being touched or tapped&brushing. It hurts even to bite on the cotton roll or qtip.Tooth and gum hurts too. Gum hurts below the tooth.

problem could be in second molar as periodontal space seems to be widened on distal aspect of last molar … get that also checked and curettage

get a cbct just to rule out a missed canal or some tooth fracture which may not be evident on iopa

I don't think so it needs an extraction...iopa is normal...
