Digestion and hard stool

Asked for Male, 68 Years

My father, who is 68 yr age and Diabitic, he is in medication for the diabetic. From last few months he is facing slow digestion and motion not get clear, and sometime he force the motion and due to hard stool he get hurt. He is worried and think lot on this as he is not finding solution. Let us know if any suggestion and diagnosis required. Thx Sathish
