The only way to distinguish between gastric problem and heart disease is to get investigations done towards heart. If you have a heart problems then some investigations will show that. Otherwise it is considered as being due to gastric issue.
In fact in the olden days there used to be a term , "Cardiac Neurosis". It implied that the patient has many symptoms pertaining to heart but all investigations are normal.
Now we don't call a patient as suffering from Neurosis but we use a milder term, "Gastric problem" .
