Constipation
The patient is diabetic from past few years. She is 80 and therefore is not physically active. She is only doing basic activities like going to washroom, bathing and eating food. She has a chronic complaint of constipation and from last few days this is getting worse. She feels the urge to go but nothing comes out. She is restless.

It can be a manifestation of long term complication of diabetes- diabetic gastro paresis. Hypothyroidism can also cause this. She needs to get tested for both. Chronic constipation can lead to formation of anal fissures which can be painful.  Other simple dietary measures like water intake, increase in dietary fibre and fruits can be adopted. Try taking lactifiber 2 scoops in water at bed time.
