Hi, I am Dr. Atul Kumar Nimawat MS ENT. The term “congested PPW” refers to congestion of the Posterior Pharyngeal Wall (PPW)—this is the back part of your throat, located behind the nasal cavity and oropharynx. It indicates inflammation, thickening, or swelling of the mucosal lining on the posterior wall of the pharynx. This congestion is usually due to persistent irritation, infection, or post-nasal discharge. Possible Causes of PPW Congestion: Chronic Rhinosinusitis (as in your case): Mucus from the sinuses drains backward and irritates the throat. This often leads to post-nasal drip, a common cause of PPW congestion. Allergic Rhinitis: Allergens stimulate inflammation in both the nasal cavity and pharynx. Gastroesophageal or Laryngopharyngeal Reflux (GERD/LPR): Acid reflux reaching the upper throat causes chronic irritation and congestion. Smoking or Environmental Irritants: Prolonged exposure to pollutants or smoke inflames the throat lining. Viral or Bacterial Pharyngitis: Acute infections can cause prominent congestion and erythema (redness). congested PPW can cause the following: Burning sensation in the nasal cavity, post-nasal region, or throat. Throat discomfort, irritation, or feeling of mucus dripping down. Frequent throat clearing or mild dry cough. Foreign body sensation (“something stuck in throat”). Occasionally, bad breath or altered voice if inflammation is significant.
Next Steps
Treatment depends on the underlying cause: Nasal Steroid Sprays – like Metaspray – reduce post-nasal inflammation. Saline Nasal Irrigation – clears mucus and allergens. Antireflux measures – if GERD is suspected (e.g., antacids, dietary control). Allergy medications – if allergic rhinitis is present. Avoid irritants – such as smoking, dust, cold air.
