Collar bone pain and shoulder pain
I experience extreme pain in my collar bone right side and back of neck and shoulders. It's difficult for me to raise my hands. The pain is unbearable.

Sorry about your pain.
It seems that you have main issue with the shoulder. You can take Zerodol P twice a day and get an x-ray done of the shoulder and upload here.
