Chondomalacia Patella and Chondral loss
On my left knee - chondromalacia patella grade 3 and moderate bone bruising and small baker's cyst On my right knee - moderate chondral loss in patellofemoral compartment. Grade 4 chondral loss in the mid medial facet. And small effusion with prominent debris. I have couple of questions - 1. How far I am from Osteoarthritis ? Or is this osteoarthritis ? 2. Can it be reversed ? If yes, how ? 3. Can I go back to sports - running, soccer , skiing etc.

The problem is patellofemoral malalignment causing the MRI picture. This happens due to weakness of the muscles of your legs and the weakening is related to lifestyle, primarily sitting. Whatever has damaged cannot be reversed but the pace of deterioration can be reduced. Once the pain is reduced and the muscles are strengthened, you can definitely return to sports which involves less of knee bending.
Next Steps
patella taping, change in lifestyle and appropriate stretching and strengthening exercises with functional training.
