Of course it should be. Sometimes it becomes a necessity when you travel abroad. If any adverse effects are seen with any particular vaccine, then it can be reported and the whole lot of vaccines can be traced and informed to the District Immunisation Officer.

It is not mandatory but yes it is recommended. It is used for reference purposes, to know name, expiry date of the vaccine and details of the vaccine.
