I understand your concern regarding diabetes. The CGM is a good tool for continuous monitoring of blood glucose. In your results your average blood sugar is 148 but your blood sugar levels peak post meal. Carbohydrates restricted diet and exercise could help but that could be assessed only over long periods of monitoring.
Next Steps
i would suggest you to just start with Metformin sustained release tablet once daily with sugar monitoring. this will improve the insulin sensitivity and decrease the HbA1C. You can consult for further assistance.

You are diabetic of sure, diet exercise and medication will bring under control. As you mentioned about medication you are on to but still sugar levels are high that means your diet is not properly controlling your sugar . You need to do more of hard work diet control for sugar control. Discipline approach will bring sugar levels down and also reduce medication

Hi, It’s difficult to stop all the medicines at once. What you can do is to take those medicines which have less chances of hypoglycaemia and continue with lifestyle modifications. Drugs like metformin, sitagliptin, DAPAGLIFLOZIN have less risk of hypoglycaemia compared to Glimepride. Dose needs to be adjusted based on your CGM profile.
Next Steps
Consult with all the reports and medicine dosage details.

Continue lifestyle modifications
