All the time I feel like I'm confused, I have mood swings, I find it hard to concentrate and stay focused
Is improving concentration possible through medication...I have migraine issue also

Yes you can do is start meditating by focusing on a point in ur room. Try controlling thought.
Relaxation exercises
Avoid electronic devices and mobile phone especially at night before bedtime.
Sleep early and wake in morning with regular exercises.
