Many test are outside reference range. anaemia is a problem, slight hypothyroidism and developing diabetes. This is coupled with deranged GFR, therefore we have to be careful while prescribing anything. Please consult and detailed assessment is necessary

Moderate anemia with Iron deficiency and b12/ vit d3 deficiency as well. Should go for iv iron transfusion. For rest oral supplements. Repeat tsh after 3 months
