Your beta hCG rising only from 1306 to 1620 in two days, together with the bleeding, clots and a scan not showing a sac anywhere, points to a pregnancy that may not be developing normally, such as an early miscarriage. Because the scan hasn't located the pregnancy yet, an ectopic pregnancy still needs to be ruled out even though none was seen this time.
Next Steps
Please see a gynaecologist in person today or tomorrow rather than waiting, given the bleeding and fever. They will likely repeat the beta hCG in 48 hours and do another ultrasound, since the trend and a clearer scan together will show what's actually happening.
Health Tips
Go to the emergency department right away if bleeding becomes heavy, you feel faint or dizzy, pain worsens sharply, or the fever climbs.
