Back pain after getting up from bed
28 years male.. Back pain both upper and lower back pain for past 1 year..paraspinal.. more after getting up from bed... Slowly decreases over the day.. but not completely relieved... Orthopaediacian took x ray spine..no changes of TB.. vit D deficiency treated.. ESR normal..currently on stretching exercises... But no relief..kindly opine.

Paraspinal backache can be due to facet arthritis or myofascial pain syndromes. X ray spine cannot pick up minute details of bone and no soft tissue evaluation is possible. An MRI of Lumbo sacral spine will be helpful.
Next Steps
Consult a pain specialist as detailed clinical evaluation can often help to reach a diagnosis. Both the above conditions can be treated with interventions like facet joint injection or Intramuscular stimulation. Once pain relief is achieved you need to undergo physiotherapy.

1. Sit with straight back and change posture in between. Do not hunch and feet should rest on floor while seated. 2. Take breaks in between work and walk. 3. Take rest when back pain is severe but a void total bed rest. 4. Do back strengthening and stretching exercises daily. 5. Take simple painkillers like paracetamol if pain is persistent. 6. Apply analgesic cream over back followed by warm fomentation. 7. Keep pillow behind your knees at bedtime. 7. Do not bend and lift weights.  8. If back pain is accompanied with radiating leg pain or tingling , numbness or weakness, consult your doctor immediately
