Since there are no warning signs such as breathing difficulty, feeding problems, or poor weight and height gain, it is likely to be a benign condition [ nothing serious , make an video following attack so to rule out breath holding spell ]. However, if you notice any of these symptoms, please consult a pediatrician or a pediatric cardiologist. They may advise a 2D echocardiography (2D Echo) if needed. Based on the limited history provided, it is difficult to give a more definite opinion.

We must get a 2 D echo done as soon as possible. After that we can discuss based on the findings.

Greetings The baby's symptoms of bluish skin and feet during crying or fever appear consistent with peripheral cyanosis or acrocyanosis, which can sometimes occur as a temporary vascular reaction to temperature changes or distress. However, because the discoloration occurred during crying in infancy and now reappears with illness, it is impossible to rule out an underlying congenital heart problem without proper diagnostic imaging. At 2.10 years old, scheduling a pediatric 2D echocardiogram remains highly recommended to definitively evaluate the structure and function of the heart. You should also consider alternative possibilities, such as a benign cyanotic breath-holding spell or a heightened vasoconscriptive response to fever.

Yes to check it out whether it is heart related you need to do a 2D Echo

From your description, it is unlikely to be a serious heart problem, as the episodes occurred only in infancy, oxygen levels were normal, and your child is now doing well. Bluish hands and feet during crying or fever are commonly due to acrocyanosis, which is usually harmless. Since the 2D echo was never done, you can still get it done once for reassurance, but it is not urgent if your child is active, growing well, and has no blue lips/tongue, breathlessness, or poor weight gain.
