Any issues on this tooth xray
I have jaw pain near ear lobe.Dentist spotted lower first molar which hurts for tapping,biting pressure on it.So he said I need RCT on it. Xrays, CBCT was clear.Had rct in 2020.Same pain/symptoms persist after RCT.It hurts for tapping,biting pressure on it.It hurts for any tapping,touching with fingers on it,rubbing with tongue on it,brushing on it hurts too.Tooth and gum hurts for any little touch or pressure on it.can't brush on it.Went to TMJ dentist,OMFS.Nothing seen. Saw second opinion Endo.He said to do retreatment on distal root alone.As he feels first RCT didn't go until the distal root tip.So I had retreatment on distal root alone in Jan'22.No relief even after 3 months.That tooth hurts for any little pressure on it.It hurts to bite on it with Cotton roll&q tip.That tooth immediately throbs for biting on it and touching /rubbing.Endo took another cbct scan and said nothing found.Do I need to extract now/wait?   They said it could be bcos of Myofascial pain. Is that possible?

The first molar seems ok ..second molar has to be evaluated clinically to rule out root caries on medical side .. also there seems to be a remaining root of a third molar. It would be helpful if you attach your OPG along with this. As i feel there is resorption of root in your premolars.

are there any triggering factors or pain is continuous..need to rule out possibilities to reach the exact cause.. there could be myofacial pain element … you can try low level laser therapy two sessions a week for 3-4 weeks if it works and i hope u have ruled out pockets and gum disease … if nothing seems working you can go for extraction but that should be the last resort

According to me this xray is ok...no issue in it...may be pain occurs due to tmj issue or may be due to ear problem...
