I am not able to study anymore...whenever i take a book to read..i am lost in my dreamy thoughts such that i waste my entire time to all the distractions. And whenever exams are approaching...i become so worried that I reluctantly don't study and it makes me want to kill my self many times because i feel i am wrecked..destroyed...useless.. I want to get out of this mess and want to have control on my decisions and persue my career of becoming a doctor... please HELP .... I really need your help.

The reason for the distractions can be various. You have to first be honest and clear of what is real reason for you to pursue your career. Once you are honest and transparent about it to your self, you energies will be directed to achieving any goal you set for as energy flows where interest navigates your attention.
Once that is done, managing the time will be easier and anxiety prior to exams can be avoided completely. All the associated labels will automatically vanish when clarity of WHY you are wanting to pursue the career u desire to.
