Anus is wet
Every time after taking food my anus becomes wet or slimy. So after going to toilet i can feel the intestine slightly out

This could be RECTAL PROLAPSE a condition in which rectum is coming out through anus due to lack muscle support to rectum. you need consultation to asses about the severity of prolapse. in initial stage , conservative treatment will work. Advanced stage , surgery will help u.
Another one more condition is prolapsed pile but this will present with ball like soft mass with or without bleeding. Anyhow u need personal examination
