**Dear Patient,**  Your symptoms of **severe anal pain without bleeding**, which is affecting your daily activities, could indicate either an **anal fissure** or **thrombosed hemorrhoids (piles)**. Both conditions require prompt attention to relieve discomfort and prevent complications.  Key Assessment:**  1. **Anal Fissure:**     - Sharp, tearing pain during bowel movements (may persist afterward).     - Often caused by constipation or hard stools.  2. **Thrombosed Hemorrhoids:**     - Intense, constant pain with a palpable lump near the anus.     - No bleeding unless the hemorrhoid ruptures.  Immediate Relief Measures:**  1. **Sitz Baths:**     - Soak in warm water for 15–20 minutes, 2–3 times daily.  2. **Topical Ointments:**     - Use a **lidocaine-based cream** (for pain) or **nitroglycerin ointment** (for fissures).  3. **Stool Softeners:**     - Take **psyllium husk (e.g., Isabgol)** or **lactulose** to avoid straining.  Medication (Temporary Relief Only):**  - **Pain Management:**    - Tab. Zerodol SP (1–2 times daily for 3–5 days).  - **Anti-inflammatory:**    - Tab. Deflazacort 6 mg (once daily for 5 days, if prescribed).  Why Consultation is Necessary:**  - A **physical examination** (proctoscopy if needed) is crucial to confirm the diagnosis.  - Untreated fissures can become chronic, and thrombosed hemorrhoids may require minor surgical intervention.  **Contact me on WhatsApp for an urgent consultation:**  **Eight Zero Zero Nine Eight Two One Three Three Two**
