Vitamin B12 277.9 is borderline/low-normal depending on the laboratory range
Next Steps
* Tab Vitamin B-complex/B12 once daily for 8–12 weeks can be considered, depending on diet and symptoms. * A General Physician can assess and manage B12 deficiency; gastroenterology review can be considered for persistent LPR.
Health Tips
* Include B12-rich foods such as milk/curd, eggs, fish, meat or fortified foods as appropriate. * Avoid unnecessary long-term acid-suppressing medication; use it only as advised. * Recheck B12 after the supplementation course. Contact me anytime on Practo for further consultation.

Your vit b12 level is not that much low that you have to worry for. If you are a vegetarian than you can take Tab vit b complex - 1 tab - once daily But if you are a nov vegetarian you can eat animal products which is a great source of vit b12. When taking medication must drink plenty of water.
Next Steps
As you are suffering from LPR(Silent reflux) must consider these things- 1. don't lie down immediately after food 2.  don't eat excessive oily, spicy, hot food 3. drink plenty of water 4. avoid tea empty stomach 5. must walk after food
Health Tips
you can go and consult rhe gastroenterologist if your reflux problem increases after all this.

Hello, Vitamin B12 level of 277.9 may be on the lower side of the normal/borderline range depending on the laboratory reference range. However, the B12 value alone is not sufficient to decide whether you need supplementation or injections. Silent reflux (LPR) itself does not necessarily mean that you have Vitamin B12 deficiency. However, certain factors such as prolonged use of acid-suppressing medicines (for example, PPIs), dietary intake, absorption problems and other gastrointestinal conditions can contribute to low B12 levels. The appropriate treatment depends on your symptoms, diet, medications, duration of reflux, previous B12 levels and other blood investigations. Depending on the situation, oral B12 supplementation may be sufficient, while injections are required only in selected cases. I would recommend that you consult me for a detailed evaluation before starting any supplementation on your own. During the consultation, we can discuss your LPR symptoms, current medications, Vitamin B12 report and any associated symptoms, and decide whether you need supplementation or any further investigations. You can book an online consultation with me on Practo, and please keep your recent blood reports and current medication list available during the consultation. Lower level of normal is not a problem unless you develop symptoms secondary to it. Like tingling sensation in your limbs, fatigue, weakness. But lower than that could lead to serious neurological effects. Increase your non vegetarian diet, green leafy vegetables if still it doesn't help then switch to meds. Worrying won't help. Get proper diet and you might feel the change there itself. For better and detailed solution do consult me on practo.

Your Vitamin B12 levels fall in the normal range if the unit is in pmol/litre.Silent reflux in itself is highly unlikely to cause vitamin B12 deficiency.
Next Steps
Avoid spicy foods, alcohol and smoking cessation if applicable You can trial a course of Tab.VONOPRAZAN 10 mg twice daily 15-30 minutes before food for 3 days along with Syp.Mucaine gel 10 ml.(This is recommended for patients not on any regular medication as medication or past medical history is not mentioned)
Health Tips
Kindly consult a clinic if you are having multiple episodes of silent reflux in a day with abdominal pain , vomiting or inability to keep solid or liquid foods down

Your B12 level of 277.9 is not clearly deficient; please confirm the units and laboratory reference range. LPR does not usually cause B12 deficiency. Avoid high-dose supplements without checking your CBC, folate and, if needed, methylmalonic acid. If you have numbness, weakness or anemia, arrange a clinical review promptly.
Next Steps
Please share the complete report and medication list with a physician for personalized advice. You can consult or book an appointment with me on Practo.

You should consult a Gastroenterologist and then a nutritionist if your daily life is getting affected. Otherwise just consume a balanced diet, fruits, fluids and do physical activity.

Start with a general physician for your B12. They can look into the cause and plan replacement for you, while keeping your blood pressure treatment in view.
Next Steps
For the silent reflux itself, a gastroenterologist is the better fit, and the two can work together. A nutritionist can help with your diet, but shouldn't be the one planning treatment. Carry your B12 report and your full medicine list to each visit.
Health Tips
Please don't choose a supplement or strength on your own. Long-term reflux treatment can affect how you absorb B12, so that choice should come from your doctor.
