Dear, Your Vitamin B12 level is 267 pg/mL, which is on the lower side of the normal range (typically 200–900 pg/mL). The choice between oral supplements and injections depends on your clinical symptoms and underlying causes of deficiency. Oral Supplements vs. Injections: 1. Oral Supplements (Tablets or Sublingual) Indicated for mild deficiency or borderline levels without significant symptoms. Suitable if absorption is intact (no malabsorption disorders like pernicious anemia, celiac disease,  gastric surgery). Requires long-term adherence for around 6-12 months (daily doses of 1000–2000 mcg). Cost-effective and non-invasive. 2. Vitamin B12 Injections (Intramuscular) Indicated for moderate to severe deficiency, neurological symptoms (e.g., tingling, numbness, memory issues), anemia( fatigue, tiredness, dizziness), or malabsorption conditions. Given as initial intensive therapy (e.g., 1000 mcg twice  a week for 4 weeks), then maintenance doses. Ensures rapid correction and direct absorption. Recommendation: If you have symptoms like fatigue, tingling, memory issues, or digestive conditions affecting absorption, injections are justified. If you have no significant symptoms, oral supplements may suffice with regular monitoring. Discuss with your doctor about your specific symptoms and underlying risk factors before making a decision

Unless there is some coexisting problem, tablets or nasal spray of B12 along with dietary management is enough to raise your B12 as long as you follow up regularly. It's your choice along with your doctor.

Initially you need to take IM injections for 2-4 weeks on weekly basis. Thereafter you should take oral 1000-1500 mcg cyanocobalamin for 1-2 months. In case of further symptoms, IM injections need to be repeated.

Oral supplements are fine unless you have any symptoms like tingling sensation, numbness, memory issues. Also you will have to adhere to the oral supplement for minimal 3 to 6 months.

A Vitamin b12 of above 300 is considered normal. So your b12 levels are on the lower side. Kindly contact Carna Health Centre Nagarbhavi Bangalore for a consultation.
