2026-05-29 18:37:01
Hi, I have been taking thyronorm 25 mcg for hypothyroidism since 9 years . My test report says T3 1, T4 8 and TSH 7.27. Should I increase dose to 37.5 mcg. 3 Months before my reports were normal but this time TSH is increased. PLEASE advice. Thanks, Juli Rai

Hey Juli, I normally joke about the Thyroid issue to my patients. It's like those radios where you had to tune them finely- you either heard clearly at a frequency or it would be just noisy disturbance.😂. That's the case with the Thyroid dose also. Your levels are definitely a bit higher and off the normal range but not concerning. Do look into the cause of this elevation: Changes in timing of medication or eating or drinking coffee/tea right after medicine or change in Diet, any new medication or supplement, you are either sitting in the roller coaster of perimenopause or done with it( this can definitely alter the levels), stress is also a contributor.
Next Steps
you are totally OK to increase the dosage of Thyronorm to 37.5mvg. In my opinion, follow this dosage for a month and repeat your Thyroid profile test on a fasting stomach from the same lab. you will be able to assess better.
Health Tips
Try to fix the intake of Thyronorm medication to early morning empty stomach , preferably before 7am.

Hi, your TSH has mildly elevated. A small dose increase is often appropriate, but it should be guided by your doctor. Don’t adjust on your own. Feel free to consult for further guidance.👍

Your TSH of 7.27 indicates that your current dose of Thyronorm 25 mcg is no longer keeping your thyroid levels in control, even though it was working well 3 months ago. This can happen due to seasonal variation, changes in weight, absorption issues, or the natural progression of hypothyroidism. A dose adjustment is very likely needed, but the exact amount should be decided by your doctor after reviewing your complete history.
Next Steps
Please consult me directly on Practo for a detailed evaluation so I can guide you on the appropriate dose adjustment.

TSH is mildly elevated. Please do not change the dose on your own. Consult for dose adjustment and repeat TSH after 6–8 weeks.

Dont increase or decrease of your own. Sometimes clinically also it has to be judged.

Thank you for reaching out , Juli Rai . Since your thyroid laboratory reports were completely normal just three months ago, we should not change your Thyronorm dosage based on a single, isolated blood test. TSH levels can occasionally fluctuate due to temporary factors like minor stress, changes in daily routine, or slight variations in how your medication was absorbed recently. Because your T3 and T4 levels are still looking normal, your body is currently getting an adequate supply of thyroid hormone. Please let me know if you are currently experiencing any Unusual or worsening fatigue and lethargy, Unexplained weight gain, Increased sensitivity to cold temperatures, Dry skin or hair thinning,or Constipation? Have you started taking any new medications, calcium supplements, or iron tablets? These can block thyroid hormone absorption if taken too close to your Thyronorm dose.Have you missed any doses recently, or changed the timing? Are you using the exact same brand of medication as before?
Next Steps
Keep an eye out for any noticeable signs of an underactive thyroid. Unusual or worsening fatigue and lethargy Unexplained weight gain Increased sensitivity to cold temperatures Dry skin or hair thinning Constipation Instead of changing the dose now, please repeat your TSH test in 4 to 6 weeks. Continue taking your current 25 mcg dose exactly as prescribed by your thyroid specialist. If you take calcium, iron, antacids, or multivitamins, take them at least 4 hours apart from your Thyronorm. These supplements bind to the thyroid hormone in your gut and prevent it from working. Ensure you take THYRONORM - FIRST thing in the morning on an empty stomach with a full glass of plain water, at least 30 to 60 minutes before breakfast, tea, or coffee. Once you repeat your TSH test in 4 to 6 weeks, please do not hesitate to share the new report with me. To ensure you get the best and most accurate guidance, I highly recommend that you connect with me personally here on Practo for a direct consultation. This will allow us to securely review both of your lab reports side-by-side, discuss any subtle symptoms you might be experiencing. Wishing you good health!
Health Tips
Thyroid hormone replacement is highly precise. Never increase or decrease your dose of Thyronorm (such as moving to 37.5mcg) on your own based on a single lab report. Changing your dose without medical supervision can cause your thyroid to become overactive, leading to complications like heart palpitations, anxiety, insomnia, or bone thinning.Any final decision to change your medication strength must be made by a physician who manages your care, after they have reviewed both your physical symptoms and a confirmatory blood test.

Hi! it is important first to confirm that you are taking the tablet correctly (on an empty stomach, 30–60 minutes before breakfast, without iron or calcium supplements nearby). A single elevated reading without symptoms should not be a marker for increasing the dose.
Next Steps
Consult your endocrinologist and get a proper prescription before changing your dosage.
Health Tips
If you have no symptoms, continuing with the same dosage and repeating the test can also be considered.
