Sudden Fear and Anxious

From childhood, I am an introvert, anxious and weak personality. Since last few years i was suffering from sudden fear and anxiety for some reasons and sometime without any reasons. But from the last few months it has now aggrevated alot and it is affecting me alot during by day to day life. I am having health anxiety, whenever i visit a clinic/hospital my thoughts speeds up and started feeling very much nervous with high heart rate and my BP was also high at that time. Now ever single day i am getting nervous for no reasons and started feeling very much anxious, tensed and uncomfortable through out the day. And i can't bear this feeling, i feel i am getting choked with these thoughts, and i am unable to concentrate on my life and as well as my carrier. I don't know what to do now, total mess up with my thoughts and anxiousness. I have tried many things to lower my anxiety, but it didn't worked out and now i am feeling helpless.
