Severe anal itching more than 2 yrs

I have severe anal itching that has been going on for more than two years. After pregnancy, I developed piles and took both Ayurvedic and allopathic medicines, and the piles are now mostly resolved. However, I sometimes still have pain while passing stool, which feels like a pin-prick, and occasionally mild bleeding. The itching is very severe both during the day and especially at night, making me extremely uncomfortable. I’m not sure whether it could be pinworms, poor hygiene, piles, a fissure, or another issue. Please guide me on the possible cause and what I should do.

Could be fluid leaking from anal canal irritating the skin .or pin worms which are more at night Proper hygiene and keep the area dry.do stool re for worms can be due to piles also

Hi If you currently don't have piles , Could be worm infestation.( Most likely). Good thing is it's treatable There is a chance of internal pile which could be causing occasional bleeding. Needs further details to come to a conclusion

The pin-prick pain while passing stool with occasional bleeding points to a chronic anal fissure, and the itching is from moisture and irritation around it - pinworms usually itch only at night and would not give that pain. Two years is long enough that it will not settle with more Ayurvedic or over-the-counter creams. What actually works: keep stools soft (isabgol at night, 3 litres water, plenty of fibre), sitz bath in warm water for 10 minutes twice a day, and a local anaesthetic-nitrate/diltiazem ointment for 6-8 weeks. Wash and pat dry, never scratch or use soap inside.
