Regarding increased CRP and RBS

Namasthe sir/madam, I'm 28 years old, I'm from vizag. Just 4 days back I was affected by cold. After 1 day fever and severe body pains also started. Then I went for tests and it came as dengue and malaria negative but typhoid with 1/80 and low tlc count of 2800 and high CRP of 19.3. so my doctor started DNS with amikacin injection and rantidine mng and night through iv . Cefpodoxime 200 twice a day, montec lc once a day and methylprednisolone 8mg twice a day. The matter is Iam non diabetic and I regularly moniter my BP and blood sugars out of my anxiety . My rbs never crossed 120. But today after checking my rbs it spiked up to 180 mg/dl. I'm very much tensed. I'm attaching my reports. Kindly respond and help me in this regard respected doctors . I can't attach my dengue report as only two pics can be attached in this. Kindly consider. Thanks in advance

Fever is due to an acute infective cause. Since total leucocyte count is low, it could be suggestive of some viral or malarial etiology, or it could also be due to some bacterial infection associated with leucopenia or neutropenia. Dengue test may not be positive on the next day of fever, so it should be repeated ideally. Antibiotics prescribed are only on empiric basis and in my opinion, higher antibiotics should be given after taking blood culture samples. As far as blood sugar is concerned, it is high because of steroid methyl prednisolone.
Next Steps
1. Blood and urine culture sensitivity testing. 2. Higher antibiotics to be given, in the light of leucopenia. 3. Methyl-prednisolone should be stopped. 4. Paracetamol to be given on SOS basis, if temperature is more than 100F.

Steroids are known to raise the blood sugar levels tremendously. You were getting Methylprednisolone 8 mg twice a day which is a heavy dose. CRP is a non specific test. So there was no need to give heavy antibiotics. Your Typhoid test was also negative. I suppose if you stop all your treatment except paracetamol and H2 receptor blockers you will become all right.

Specific diagnosis has not been confirmed.Your widal test titre is insufficient to conclude you have typhoid .Needed follow up to observe raising/falling titre. Your doctor has given empirical therapy of antibiotics. Your symptoms giving suspicion of viral infection.And dengue serology is mostly negative on 4-6th day( both antibody and antigen may come negative)of fever even if you are positive. Your CRP is raised,in acute infection blood sugar may rise.You have not mention if it was fasting blood sugar or 1hr after breakfast/Lunch.
