Urine report suggestive of urinary tract infection. Urea and creatinine levels are also quite high suggestions some kidney damage. Please let us know if the patient have diabetes? Esr is high which shows inflammation or infection in the body. There is sever vit d and vit b12 deficiency and mild anemia. Intervention is needed. Consult for proper evaluation and management.
