Hello sir...
pain and sensitivity may be due to 2 reasons..
1..Erupting 3 rd molar on both sides pushes the 2 nd molar and cause erosion of enamel and dentine of that tooth and it is approaching the puip which cause sensitivity and pain
2.The impacted lower right 1 st premolar which give pressure to the reained primary teeth also cause sensitivity and pain..
MANAGEMENT:
1..you need to go for extraction of 3 rd molar and go for filling with subbase of
calcium hydroxide on right 2 nd molar and root canal treatment of left 2 nd molar as the erosion approches the pulp...
2.. removal of retained primary teeth and orthodontic eruption of permanent premolar.....
