Is this lactose intolerance self diagnosed or medically determined. Needs further evaluation and management

Likely lactose intolerance. Avoid/reduce milk and use lactose-free dairy or alternatives. If accidentally consumed, lactase enzyme with the meal may help; symptoms usually settle on their own. Persistent/severe symptoms → consult a physician.

If accidentally you take milk anytime, drink more water or electrolyte solutions, take tab Gasex to reduce bloating, tab loper amide 2mg ,if diarrhoea occurs. You can apply warm water bottle to stomach to reduce painful bloating. Take bland food.

If you accidentally consume dairy, take a lactase enzyme tablet with/just before dairy. If symptoms develop, take fluids and avoid further dairy until settled. Prefer lactose-free milk/curd or small quantities of tolerated dairy. If symptoms occur even with lactose-free products, please consult a doctor to rule out other causes.

If you are having lactose intolerance and happen to have accidentally taken milk or diary products, just take an adequate amount of water to flush out the ingested milk. There is no other alternate options. And kindly avoid any diary or milk products for 2-3 weeks. Try with lactose free supplements.

Avoid milk altogether and dairy items too. There are a lot of good alternatives available these days. If you happen to accidentally have dairy items, have more fluids to flush out the dairy and have light food till lactose moves through the gut. Nothing much can be done.
Next Steps
Lactase-enzyme can be helpful but wont instantly reverse the symptoms.
Health Tips
I will advise you to have food with caution if your symptoms are very distressing after dairy intake.

This sounds most consistent with lactose intolerance, especially if symptoms recur after milk or dairy. It is uncomfortable but usually manageable, and it is not the same as a milk allergy. I would also consider other causes if fatigue or anxiety persists even without dairy.
Next Steps
Avoid milk for 2–3 weeks and choose lactose-free milk or curd if tolerated; accidental intake is usually managed by stopping the trigger and observing symptoms. If pain, vomiting, breathing difficulty, rash, blood in stool, or persistent weight loss occurs, seek care promptly. Book a Practo consultation or visit me at Synergy Multispeciality Hospital, Miraj for assessment.

Lactose intolerance causes exactly this pattern, bloating, low energy and discomfort after dairy. It's very manageable once you know your own triggers.
Next Steps
Try cutting dairy for two weeks and reintroducing it slowly to see your own tolerance level. A lactose intolerance test can confirm it if symptoms keep recurring.
Health Tips
Lactase enzyme supplements before dairy can help some people. See a doctor if the anxiety or fatigue continue even when you avoid dairy completely.
