Levera to Briv Switch

2026-09-15 08:46:42
For past 5 years, I was on Levera 500 mg twice a day. But due to side effects like low concentration, emotional numbness, low mood, lack of motivation etc, my neurologist has switch medicine to Brivaracetam 50 mg in the day and 100 mg in the night. I have been on Briv since a week now. How long does it take to experience benefits of switching from keppra to Briviact?

In Another 3-4 week you will get rid of side effects of Levicetram.

Brivaracetam starts working relatively quickly because it reaches effective blood levels soon after starting treatment. However, improvement in seizure control and tolerability can vary from person to person. Since you have been taking it for only one week, it may be too early to judge the full benefit of the switch. Next Steps Continue brivaracetam exactly as prescribed by your neurologist and maintain regular dosing without missing doses. Keep a record of seizures, mood, concentration, sleep and any new side effects, and discuss these during follow-up. Your neurologist can adjust the dose or treatment if required.

Levetiracetam a generally considered as the first line of anti epileptic drug. If you are having symptoms and shifted to brivaracetam. Levetiracetam is associated with behavioural changes and the other drug which is brivaracetam more selective and have near to negligible behavioural changes as compared to levetiracetam. You should continue this, and usually start seeing changes more, evidently after 2-3 weeks

If a patient on levetiracetam has: ✅ No breakthrough seizures ✅ Good seizure control for the intended duration ✅ No significant adverse effects, especially behavioral/psychiatric effects ✅ Good adherence ✅ No clinically important drug interactions ✅ Renal function/dose are appropriate then there is generally no need to switch levetiracetam to brivaracetam simply because brivaracetam has higher SV2A affinity. Practical approach LEV working well + tolerated well → Continue LEV. Consider switching to brivaracetam mainly when there is a reason, such as: behavioral adverse effects with LEV inadequate seizure control despite appropriate LEV therapy tolerability problems a specific clinical reason to prefer BRV. Higher SV2A affinity ≠ automatically better clinical efficacy. So in a stable, seizure-free patient who is tolerating levetiracetam well, changing an effective ASM can itself introduce unnecessary risk of adverse effects or loss of seizure control.

Hi Switching from Keppra (levetiracetam) to Briviact (brivaracetam) often provides rapid physiological transition, but noticing improvements in mood or seizure control can take anywhere from a few days to a couple of weeks. You are already on right dosage of medication. Continue the same. Wait for reversal of your symptoms. If persistent even after couple of weeks, look for other causes for your symptoms

Brivaracetam is a reasonable switch for Keppra-related mood/concentration side effects. Improvement may occur within 1–4 weeks. Continue as prescribed and follow up with your neurologist

Both medicines are closely related , since u have been on briva since a week ,its early to judge the improvement,  but yeah some side effects may improve

Brivaracetam is closely related to levetiracetam, and the medicine reaches effective levels quickly, but seizure control and improvement in side effects may take a few weeks. The switch and dose should be guided by your neurologist; do not stop or alter either medicine yourself.
Next Steps
Continue the prescribed Brivaracetam at the same times and track seizures, sleep and mood symptoms over the next few weeks. Seek urgent care for a prolonged seizure, repeated seizures without recovery, or severe mood changes. For a medication review, book a Practo consult or visit Synergy Multispeciality Hospital, Miraj.
