High level of tsh and low t4

Recently my wife had a medical checkup and found that tsh is 83.3 which is way higher than normal and t4 is 3.9 which is less than normal range. She often complains about body pain, dry hair and lethargy. Vitamin D levels are normal though. Can you please suggest what should be the treatment for this and how to treat it naturally as well. Which speciality doctors should we consult from.
