Hi Your son is 14 years, weight is 51kg.... Even if you consider paracetamol of 15mg/kg that also comes around 750gms. Instead you can give 500mg tablet or 650mg tablet orally instead of syrup.. It is always good to treat root cause of fever rather than choosing paracetamol or mefenamic acid
Next Steps
feel free to consult me on practo for further evaluation and management

At 51 kg your son should be on a proper paracetamol dose, not syrup - 500 to 650 mg tablet every 6 hours when fever is above 100, maximum four doses a day. Dolo 250 syrup 10 ml gives only 250 mg, which is why the fever keeps coming back; that is an underdose for his weight. I would avoid Meftal-P (mefenamic acid) in a child with fever - it commonly upsets the stomach and is not needed when paracetamol is dosed correctly. Give sponging and plenty of fluids alongside.
Next Steps
If fever crosses 3 days, get CBC with dengue and malaria tests done. Feel free to consult me on Practo with the reports and I will guide the treatment.

Since your son is 14 years old and weighs 51 kg, the dose of paracetamol should be calculated according to his weight. Dolo (paracetamol): The usual dose is 10–15 mg/kg per dose. For 51 kg, this corresponds to approximately 500–750 mg per dose, with an appropriate interval between doses. So Dolo 250 syrup 10 mL may be an underdose depending on the concentration of the syrup. Please check the strength written on the bottle before giving a higher dose. I would not advise switching to Meftal-P routinely or alternating it with paracetamol without assessing him first. For now, keep him well hydrated, dress him comfortably, and focus on how he is feeling rather than trying to bring the temperature completely to normal. Since he has fever, I would also like to know how high the fever is, for how long, and whether he has symptoms such as cough, sore throat, vomiting, loose stools, headache, abdominal pain, rash, breathing difficulty or urinary symptoms. Please consult me on Practo for an online pediatric consultation, so I can assess his symptoms and advise the appropriate medicine and dose safely.
