It is nothing but muscle strain in 90% cases. Take a hot water compress for 15 minutes. Instantly you will get relief from pain. If there is no relief then we have to consider other causes.

Pain on deep breathing can be due to a muscle strain, but pleurisy or a lung/chest problem must be ruled out—especially if it is new. Please tell me if you have fever, cough, breathlessness, injury, or pain worsening with movement. If you develop breathlessness, chest pressure, sweating or sudden worsening, seek emergency care now. Otherwise, book a Practo consult or visit me at Synergy Multispeciality Hospital, Miraj.

Pain that's worse specifically when you breathe in deeply is often muscular or related to the chest wall, but it needs to be checked to rule out anything in the lungs.
Next Steps
Please see a doctor soon, especially if you also feel breathless, feverish, or the pain is getting worse.
Health Tips
Don't ignore new breathlessness or chest tightness alongside this pain.

Pain in the upper back that worsens with deep breathing can have several causes, most commonly muscle strain or irritation of the chest wall, especially after exercise, prolonged posture, or a sudden movement. However, since the pain is related to breathing, other causes involving the lungs or pleura should also be considered. If the pain is mild and you otherwise feel well, you can rest, avoid strenuous activity, and use a warm compress. Avoid taking painkillers without considering your medical history and other medications. Please seek prompt medical evaluation if you develop breathlessness, chest pain, fever, persistent/worsening pain, cough, coughing blood, dizziness/fainting, palpitations, or sudden severe pain. If the pain persists despite rest or you are unsure about the cause, an in-person examination is advisable.

It can be due to Muscle strain – more likely if it hurts with twisting, moving the shoulder back, or pressing the area. It can also be Pleurisy – sharp pain that gets worse with deep inspiration or coughing, sometimes after a viral/bacterial infection. OR. lung infection ,tell me if you also have fever, cough, sputum, breathlessness, or was it sudden!?? Consult immediately If you have worsening shortness of breath ,Chest pain, dizziness, fainting, fast heart rate, coughing blood, sudden severe worsening of pain, or any swelling or pain with the breathing If none of these are present, and you’re otherwise feeling well, it is less likely to be something dangerous. You can monitor it for a short period, avoid strenuous activity, and see whether movement or pressing the area reproduces the pain.
