ARV administered on gluteal region
I started my day 0 dose of five dose ARV schedule for dog bite on 23rd of june and it was given on the gluteal region . Later i read somewhere that it should only be given on the arm on deltoid region. Please help me understand whether i need to repeat my day 0 dose on arm or if i can continue with next dose onwards correctly on deltoid region
No need to repeat the Day 0 dose. While the deltoid (arm) is the preferred site for anti-rabies vaccine (ARV) in adults, a dose given in the gluteal region is still effective â the vaccine reaches the bloodstream and produces immunity regardless of the injection site. The WHO guidelines prefer deltoid to avoid the thicker fat layer in the gluteal region which can sometimes slow absorption, but studies show the gluteal site still provides adequate protection. Simply continue your remaining doses on the deltoid as planned.

Intra muscular dose is recommended for ARV dosing in some patients if they have a very thin deltiod muscle we can advice them to take ARV in gluteal region.that doesn't make any difference whether you take in deltoid or gluteal .

You are correct. ARVs should never be injected into the gluteal muscle (buttocks). Fatty tissue in this region slows absorption, leading to a weaker immune response. As rabies is 100% killer, so you need urgent medical attention.

If a rabies vaccine has been given in the buttocks, you must seek immediate medical care to have the dose repeated in the correct anatomical location (the deltoid muscle of the arm or the thigh).

According to who rabies vaccine given in deltoid region in adult and in small children anterolateral area of thigh.
Rabbies vaccine should not be given in gluteal region because of low absorption due to adipose (fat) tissue

It's not necessary. As you said it's five dose of ARV.. 5 doses comes for IMRV ( intramuscular ) we have 2 type regimen for ARV intradermal which we give on both deltoid and intramuscular which we can take for muscle. Either deltoid or gluteal.. so no need to worry about it.. just continue the same dose as per schedule
