If you are sure that it is addiction, you definitely need to consult psychiatrist first to manage withdrawal symptoms. Withdrawal symptoms can vary from simple with drawl to complex withdrawls.
Once detox is done therapy is advisable.
Also need physical examination, blood investigations.
